Self Scheduling
Established patients can self-schedule or reschedule appointments. Please be aware there is a $150 fee for cancellations or rescheduling less than 24 hours in advance for initial evaluations and a $50 fee for follow up appointments.
*You can schedule telehealth from both the Whitehall and Irwin office links. Even though we're meeting virtually, you need to be connected to a physical office location.
